For a book about the psychology of accidents and disasters, Casner is much more interested in shaming people for not being careful enough than he is in investigating what it is about human psychology that makes it so. The first seven chapters or so, when he talks in generalities, are all right (although his assertion that there is literally nothing left to do structurally about making the world a safer place seems short-sighted to me), but when he gets into specifics it becomes downright nasty.
