Weirdness on adding a book

When I added a book just now, the following appeared above the new book on the Add Books page:

NOTE! I added a second book and this did not appear.
ETA: If it helps, the books was History of Immigrant Female Students in Chicago Public Schools, and the source I used was Washington State University.
